Intake Forms

Prior to your first appointment, fill out the new client intake form. The consultation agreement will immediately follow when you finish the intake form. Please complete 48 hours before your first appointment.

Types of appointments

Complete chronic care

Constitutional or chronic care homeopathy affects the body on multiple levels because it treats the deepest imbalance of your system. In the complete appointment, I will consider all of your symptoms, each body system, your medical history, your personality and temperament. This allows me to understand you deeply, and find the remedy that is best for you, a remedy that addresses the most fundamental cause of your symptoms.  Once found, this simillimum (remedy most similar to you) strengthens the immune system, alleviates physical symptoms, and balances your emotional and mental state.

Focused chronic care

Similar to the complete chronic care model, we are looking for a remedy that addresses your symptoms at a fundamental level. In a focused consult I consider only one chronic problem or symptom, along with your medical history, your personality and temperament. This is a good choice if you are a person who only has one main complaint, but it is chronic and you would like resolution.

Acute support

I offer care to my current clients for issues that come up unexpectedly; acute illnesses and infections, traumas and injuries. I am usually available on the same day for a consult for anything you might need to discuss regarding your health. I respond to emails very quickly to let you know my availability.

Complete Chronic Care – 3 hours initial appointment: $475
Focused Chronic Care – 2 hours initial appointment: $320
Follow up appointments – 1 hour: $130
Acute support: $35/15 minutes
After-hours acute support: $45/15 minutes

Checks, cash, PayPal, Zelle, Venmo and credit card. Credit card payments incur a 5% fee.
