Nature of Work Performed by Practitioner:
I understand that my practitioner evaluates my entire condition based on a holistic, homeopathic approach, and seeks to assist me to stimulate my body’s own healing mechanisms with the use of substances prepared according to the guidelines of the Homeopathic Pharmacopoeia of the U.S, as regulated by the FDA. I understand that my practitioner may also discuss with me the use of other integrative therapeutics to improve my health, and that these are within her scope of practice to the extent that she incorporates them. I agree that I am interested in enhancing my own abilities to establish health in mind and body.
Training and Credentials of Practitioner:
I have reviewed the training and credentials of the practitioner listed below. I understand that my practitioner is not a medical doctor, has not presented herself as such, and does not seek to diagnose, treat, or prescribe for disease, disorder or other pathological conditions, and that she provides health consultation services under the California Business and Professions Code sections 2053.5 – 6.
About Alexis White, LHP, CCH, MFA:
Alexis White attended undergraduate and graduate school at UC Berkeley, UC Santa Cruz, The School of Visual Arts and Art Center College of Design. She graduated from a 3-year program at the Pacific Academy of Homeopathic Medicine and Berkeley Institute of Homeopathy in San Francisco and Berkeley, California in 2009, with additional years of training and clinical work at BIH and The Other Song Academy in Mumbai, India. She is nationally certified in homeopathy by the Council for Homeopathic Certification. Alexis is a licensed homeopathic practitioner through the Arizona Board of Homeopathic and Integrated Medical Examiners in the state of Arizona.
Cost of Consultation:
I have reviewed the fee statement below. I agree to pay these fees at the time that services are provided by cash, check, credit card, Venmo, Zelle or PayPal. I agree to the office policies and charges as described in the fee statement.
Professional Conduct and Consultation:
Practitioner agrees to honor confidentiality and assures professional conduct as defined by the Code of Ethics of the Council for Homeopathic Certification; Client grants permission for my practitioner to discuss details of his/her health in conferral with professional colleagues or use client’s case as a case study in a professional setting without additional confidentiality waiver. This agreement becomes part of client’s case records. Client agrees to consult a licensed physician for any medical concern that now exists or arises at any time during the term of this agreement, and to inform Practitioner of any physician’s assessment in so far as it applies to the client’s work with Practitioner.
Scheduling and confirmations
I have an automated scheduling system. You will receive automated email and text reminders when the appointment is made, and then twice prior to each consult to confirm the date and time of the appointment. When you receive an appointment confirmation request, please respond. It helps me to know that you are coming. If you are not receiving these reminders, please let me know.
Cancellations
I understand that plans change, and unexpected circumstances arise that can disrupt your schedule and require a change of appointment. If it is necessary to cancel or reschedule an appointment, notification must be made at least 24 hours in advance. I will work with you to move the appointment to a new time and date. Appointments that are missed or cancelled late represent time that was scheduled for you and cannot be rebooked. Each client is offered one “first miss” or “first late cancel” that is not charged. A second or later event will be charged at the full fee.
Payment
The client is responsible for payment of all fees at time of service. Payment may be made with cash, check, credit card, Zelle or Paypal. Payments by credit card incur a 5% fee.
A $40 fee will be charged for any check returned to this office unpaid.
Consultation Fees
Complete chronic care: 3 hr – $525
Re-take Case (after 2 years) – $350
Focused chronic care: 2 hr – $350
Follow up appointment: 1 hr – $145
Acute support – $40/15 min; Monday through Friday; 9-5.
After-hours acute support – $50/15 min; includes evening, weekends and holidays.
Consult cost includes all homeopathic remedies (unless a special order is necessary), call-in time support, and brief check in calls as required to track response. Any additional supplements and special-order homeopathic remedies are charged if provided by my office or can be obtained independently by the client. Client is responsible for shipping costs.
On call office hours
I am on call for you during my office hours M-F from 8:30-9 AM. You can reach me during that half hour at no charge by calling my office (415) 457-3225. You might reach my voicemail when you call in. Leave a message and I will return the call. On holidays and my vacation days, there is no on call time. If you have not received a call back, it may be one of those days. Please leave a message, and I will call as soon as I am able.
On call issues
On call issues might be questions about remedy instructions, a brief status check-in, a very quick acute, or maybe you just need some support for something that’s come up for you. The call should take about 5 minutes. If it is clear that you need more time, or your issue is not a good match for a quick call-in, we will schedule time later that day for you.
Phone, text and email policies
I check my voicemail, email and texts daily during weekdays. I do not always check on weekends. If you have an urgent situation that cannot wait until the next on call time, please text or call my cell phone. After hours rates will always apply. If you have an urgent medical situation and cannot reach me, please call your doctor or go to your nearest Urgent Care or Emergency Room.
Charges for phone calls, texts and emails
Time spent on responding to calls, emails and texts will be charged at my acute call rates. I will accumulate charges for time spent and bill you at your next appointment or when it reaches one hour of time, whichever comes first.
Medical
Homeopathy is not a substitute for conventional medicine, nor is your homeopath a substitute for your primary care physician. I encourage you to have an ongoing relationship with a primary care physician, with whom I can partner in your healthcare.
Complaints and recourse:
Alexis is certified in Classical Homeopathy by the Council for Homeopathic Certification (CHC), a national accredited credentialing board. This certification requires that each practitioner abide by CHC guidelines for professional and ethical conduct. As with any of the healing professions, each client must have a mechanism of public recourse in the event that he or she feels that unethical or unprofessional interactions have taken place with the certified practitioner. For certified homeopaths, the mechanism of recourse is through the CHC.
The following is a statement from the CHC:
• Each CHC certificant represents the homeopathic profession in the eyes of the public and is expected to uphold the highest standards of professional conduct as described in the CHC Code of Professional Ethics and the Client/Patient Healthcare Rights.
• Any client who believes he or she has a valid complaint regarding a CHC credentialed homeopath may submit a letter to the CHC office describing his or her concerns in detail.
• Following receipt of the letter, a member of the Standards and Ethics Committee informs the complainant in writing (through postal or email correspondence) within 10 business days that the complaint has been received and requests an interview to discuss the nature and veracity of the complaint.
• The Standards and Ethics Committee notifies all parties concerned, conducts interviews, and thoroughly investigates each complaint received.
